Responsibilities

• Evaluates patients at UI Health sites of care.
• Coordinates transplant care from referral to waitlist and living donor care.
• Documents pre-transplant coordination and education per UNOS/CMS/UI Health.
• Orders testing for evaluation and reviews results with providers.
• Develops and communicates the patient's plan of care.
