Job summary

UI Health is seeking a Clinical Nurse Consultant II (Transplant) in Chicago, IL. The role coordinates pre- and post-transplant patient care, including donor coordination, education, and UNOS CMS compliant processes.

You will practice under transplant surgeons/physicians and lead education for staff while delivering high-quality patient care. The position requires BSN or higher, RN licensure in Illinois, and 2+ years in transplant or related areas, with strong communication and teaching skills.

Qualifications

• Must be a registered nurse with current Illinois licensure.
• BSN or higher from an accredited program.
• Minimum two years of nursing experience in transplant or related area.
• English communication skills (written and verbal) required.
• Current BLS/CPR and unit-specific certifications required.
• ACLS certification or within 6 months of hire.
• ANCC certification or enrollment within 6 months of hire.

Responsibilities

• Evaluates patients at the UI Health site of care.
• Coordinates care for transplant recipients from referral to waitlist and donors.
• Ensures pre-transplant coordination and education documentation complies with UNOS/CMS/UI Health policies.
• Orders testing for evaluation and reviews results with the provider per UNOS/CMS.
• Develops, coordinates and communicates patient care plans based on assessments.
• Collaborates with multi-disciplinary team to improve patient outcomes.
• Implements orders and documents in the EMR.
• Engages with patient/family to develop an education plan.
• Adds patients to UNOS waiting list and sends required letters timely.
• Coordinates and schedules living donor transplant surgeries.
