Job summary

Methodist Hospital is seeking a Post Transplant Coordinator RN to lead and coordinate care for patients with chronic organ failure, end-stage disease, LVADs and transplants. You will work with the transplant team to plan and implement patient care, ensuring optimal outcomes and smooth patient transitions.

The role requires an RN with a current license and at least 2 years of critical care experience. LVAD experience is preferred. BLS must be completed within 30 days of starting employment.

Qualifications

• RN with active licensure in good standing.
• Minimum 2 years of critical care experience in a hospital setting.
• LVAD experience preferred.
• BLS must be obtained within 30 days of employment start date.

Responsibilities

• Assess, plan, implement, and evaluate care for transplant patients.
• Coordinate care delivered by the transplant team to affect positive outcomes.
• Collaborate with multidisciplinary teams to support end-stage organ disease patients.
