BayCare Health System in Tampa, FL, is expanding its solid organ transplant program and seeks a Transplant Licensed Social Worker to support patients at St. Joseph's Hospital.

You will collaborate with physicians, nurses, and care teams to assess readiness, plan care, promote adherence, and provide counseling throughout transplant—from evaluation to recovery. This full-time hybrid role emphasizes interdisciplinary teamwork and compassionate patient-centered care.

Qualifications

• Master's Degree in Social Work (MSW) required.
• FL LCSW licensure required.
• Minimum 2 years of licensed social work experience in transplant services.
• Excellent communication and collaboration with interdisciplinary teams.

Responsibilities

• Conduct psychosocial evaluations to assess transplant readiness.
• Evaluate social support, adherence, and risk factors.
• Develop individualized care plans and coordinate with care teams.
• Provide counseling to reduce anxiety and promote adherence.
• Help patients navigate resources and post-transplant adjustment.
• Contribute to quality improvement initiatives and protocol development.

BayCare is taking a significant step forward in expanding access to specialized care by developing a solid organ transplant program. We are collaborating with Northwestern Medicine and this strategic relationship will help accelerate this high-quality transplant program. This initiative reflects BayCare's continued focus on advancing complex, lifesaving care while strengthening our role as a leading academic health system in West Central Florida. This Transplant Licensed Social Worker will work directly with transplant patients at St Joseph's Hospital in Tampa, FL.

The Transplant Licensed Social Worker responsibilities include:

• Act as member of collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community
• Psychosocial Assessments:
• Conduct thorough psychosocial evaluations to assess a patient's readiness for transplant surgery, recovery and rehabilitation
• Evaluating social support systems, family stability, adherence to medical advice, lifestyle behaviors and understanding of transplant risks and benefits.
• Identify potential barriers to transplant eligibility and help develop individual care plans.
• Provide ongoing counseling and resources to promote adherence to medication regimens, lifestyle adjustment and emotional well-being.
• Provide support to reduce anxiety, depression and potential substance misuse, improving long-term outcomes.
• Quality Improvement Duties:
• Develop clinical protocols

Certifications and Licensures:

• Required FL LCSW (Social Work)

Education:
