Job summary

AdventHealth in Orlando, FL is seeking a capable transplant social worker to perform comprehensive psychosocial assessments and collaborate with the multidisciplinary team. You will educate teams on psychosocial aspects of illness, participate in transplant and VAD processes, and provide ongoing support to patients and families throughout the transplant journey.

Must possess knowledge of Medicare/Medicaid rules and labor with a strong focus on crisis intervention, discharge planning, and quality

Qualifications

• Excellent interpersonal and written communication skills are required.
• Ability to work autonomously while collaborating with the team is required.
• Ability to assess physical and emotional impact of transplant and advanced therapies.
• Ability to complete psychosocial assessments across age groups and plan discharge and care.
• Knowledge of governmental agencies, resources, and patient assistance programs.

Responsibilities

• Perform comprehensive psychosocial assessments for transplant patients and families.
• Collaborate with the multidisciplinary team to establish goals and treatment plans.
• Educate team on psychosocial aspects of illness, disability, and hospitalization.
• Coordinate transplant education, orientation, and post-transplant support.
• Facilitate crisis intervention, caregiver concerns, and referrals as needed.
• Lead onboarding and professional development for team members and stay updated on regulations.
