Job summary

BayCare is expanding its transplant program at St. Joseph's Hospital in Tampa, FL. We seek a Transplant Licensed Social Worker to work within a collaborative care team, conducting psychosocial assessments and developing patient-centered care plans.

The role requires an FL LCSW license, a Master of Social Work, and at least 2 years’ transplant experience. Responsibilities include counseling, barrier identification, and protocol development in a hybrid work setting.

Qualifications

• Required FL LCSW (Social Work).
• Master of Social Work degree required.
• Experience working in transplant services is preferred.

Responsibilities

• Conduct psychosocial evaluations to assess transplant readiness and rehabilitation.
• Evaluate social support, family stability, adherence to medical advice and transplant risk understanding.
• Identify barriers to transplant eligibility and develop individualized care plans.
• Provide ongoing counseling to promote adherence to medications and emotional well-being.
• Help reduce anxiety, depression and potential substance misuse to improve outcomes.
• Contribute to quality improvement by developing clinical protocols.

BayCare is taking a significant step forward in expanding access to specialized care by developing a solid organ transplant program. We are collaborating with Northwestern Medicine and this strategic relationship will help accelerate this high-quality transplant program. This initiative reflects BayCare’s continued focus on advancing complex, lifesaving care while strengthening our role as a leading academic health system in West Central Florida. This Transplant Licensed Social Worker will work directly with transplant patients at St Joseph's Hospital in Tampa, FL.

The Transplant Licensed Social Worker responsibilities include:

• Act as member of collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community
• Psychosocial Assessments:
• Conduct thorough psychosocial evaluations to assess a patient's readiness for transplant surgery, recovery and rehabilitation
• Evaluating social support systems, family stability, adherence to medical advice, lifestyle behaviors and understanding of transplant risks and benefits.
• Identify potential barriers to transplant eligibility and help develop individual care plans.
• Provide ongoing counseling and resources to promote adherence to medication regimens, lifestyle adjustment and emotional well-being.
• Provide support to reduce anxiety, depression and potential substance misuse, improving long-term outcomes.
• Quality Improvement Duties:
• Develop clinical protocols

Certifications and Licensures:
