Job summary

AdventHealth in Orlando, FL is seeking a capable transplant social worker to perform comprehensive psychosocial assessments and collaborate with the multidisciplinary team. You will educate teams on psychosocial aspects of illness, participate in transplant and VAD processes, and provide ongoing support to patients and families throughout the transplant journey.

Must possess knowledge of Medicare/Medicaid rules and labor with a strong focus on crisis intervention, discharge planning, and quality

Qualifications

• Excellent interpersonal and written communication skills are required.
• Ability to work autonomously while collaborating with the team is required.
• Ability to assess physical and emotional impact of transplant and advanced therapies.
• Ability to complete psychosocial assessments across age groups and plan discharge and care.
• Knowledge of governmental agencies, resources, and patient assistance programs.

Responsibilities

Job Description:

Other duties as assigned. Performs comprehensive psychosocial assessments, including evaluating patient’s understanding, motivation, mental status, employment, disability, insurance coverage, compliance, social history, substance use, functional status, caregiver, and transportation plan. Collaborates with the multi-disciplinary team to establish and document patient goals and treatment plans. Serves as a consultant and educator to the multi-disciplinary team regarding psychosocial components of illness, disability, and hospitalization. Participates in initial patient orientation and provides ongoing education on transplant, living donation, and VAD processes. Utilizes advanced competence to perform crisis intervention, including follow-up to assess adjustment, caregiver concerns, mental status, and making referrals as necessary. Assists in patient advocacy, resource brokering, and information and referral services. Provides support and education surrounding end-of-life issues and advance directives. Facilitates ongoing support group meetings for transplant patients and their families. Attends and participates in transplant team meetings, including patient review committee, hospital interdisciplinary rounds, discharge planning, and Quality Assurance Process Improvement (QAPI). Proactively initiates quality improvement ideas to improve patient care experience, health of populations served, and reduce clinic care costs. Assists with onboarding, training, and professional development of LCSWs and coordinates learning opportunities for team members. Maintains current knowledge of Medicare, Medicaid rules, and legislation affecting transplant patients.

Knowledge, Skills, and Abilities:

• Excellent interpersonal and written communication skills. [Required]

• Ability to assess physical and emotional impact of transplant, VAD and or other advanced therapy on patient and patient’s family support system; ability to assist patients/families in problem solving and in planning for transplant and post-transplant needs [Required]
• Ability to complete and document comprehensive psychosocial assessments on pediatric, adolescent, adult, and geriatric patients; ability to assess discharge and care management needs [Required]
• Knowledge of state and federal governmental agencies as they relate to healthcare; knowledge of state and community resources; knowledge of patient assistance programs, funding sources, and eligibility criteria [Required]
• Knowledge of heart, lung, kidney, pancreas, and liver diseases; knowledge of mental disorders, and substance use/abuse issues [Required]
• Knowledge of end-of-life issues and palliative care needs; knowledge of loss issues/bereavement [Required]
• Knowledge of theoretical frameworks such as Cognitive Behavioral Theory, Solution-Focused Theory, and Systems Theory. [Required]
• Transplantation [Preferred]
• Bilingual (English required/Spanish preferred) [Preferred]

Work Experience:

• 2+ experience in case management, healthcare social work and/or mental health counseling [Required]
• 2+ transplant experience [Required]

Licenses and Certifications:

• Clinical Social Worker License (LCSW) [Required]
• Clinical Transplant Social Worker (CTSW) [Preferred]

Pay Range:

$54,766.40 - $101,878.40
