Job summary

UW Health in Madison, WI seeks a Clinical Pharmacist specialized in transplant to join the interdisciplinary team. You will manage medication therapy for transplant patients both inpatient and ambulatory, ensuring safety, efficacy, and cost-effectiveness.

The role includes training residents and students and supporting a diverse transplant population. The ideal candidate has an ASHP-accredited residency and Wisconsin licensure within 90 days, with 1 year of clinical care experience and BLS

Qualifications

• Bachelor's Degree in Pharmacy or PharmD required.
• ASHP Accredited Residency preferred.
• Licensed to practice in Wisconsin within 90 days required.
• BLS within 90 days required.

Responsibilities

• Provide medication therapy management as part of the transplant team for inpatient and ambulatory patients.
• Perform clinical and distributive tasks to ensure safe, effective, timely therapy.
• Provide training and education to Pharmacy Residents and PharmD students.
• Care for kidney, liver, pancreas, heart, and lung transplant patients.
