Job summary

UW Health in Madison, Wisconsin is seeking a Clinical Pharmacist - Transplant to join the interdisciplinary transplant team. You will provide medication therapy management for inpatient and ambulatory patients and perform clinical and distributive tasks to ensure safe, effective, and timely drug therapy.

The ideal candidate has completed an ASHP-accredited PGY2 Transplant Residency and will contribute to training Pharmacy Residents and PharmD students while caring for kidney, liver, pancreas,

Qualifications

• Bachelor's Degree in Pharmacy or PharmD required.
• ASHP Accredited Residency training preferred.

Responsibilities

• Provide medication therapy management as part of the transplant team for inpatient and ambulatory patients.
• Perform clinical and distributive tasks to ensure safe, effective, timely and efficient therapy.
• Provide training and education to Pharmacy Residents and PharmD students.
