Job summary

Baylor St. Luke’s Medical Center in Houston seeks an RN Transplant Coordinator to orchestrate the transplant process from referral to recovery. You will manage referrals, evaluate candidates, coordinate in-hospital care, and educate patients and families while maintaining open communication with payers and the care team.

Qualifications

• RN licensure in TX required at hire.
• 3+ years of experience required at hire.
• Acute Care/ICU and/or transplant experience required or preferred.

Responsibilities

• Initiate referral and evaluation for transplant candidates and prepare data for Medical Review Board.
• Lead day‑to‑day in‑hospital patient care and participate in rounds with the care team.
• Communicate with referring physicians and payers to prevent denials.
• Oversee pre‑op and post‑op education, addressing language barriers and patient needs.
• Provide staff and community education on transplant and organ donation.
• Collaborate in research protocols and patient education materials.
