Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse in Minnesota to ensure a seamless patient care continuum across the transplant process. You will coordinate services, educate families, and lead quality initiatives in collaboration with a multidisciplinary team.

Qualified candidates hold a BSN from an ACEN/CCNE-accredited program, RN licensure, and 1 year of RN experience (3 years preferred); BLS certification is required.

Qualifications

• Graduate of a baccalaureate ACEN or CCNE accredited nursing program.
• Minimum 1 year of clinical RN experience required, 3 years preferred.
• Must be able to work flexible hours and adapt to unpredictable situations within the patient care setting.
• Demonstrated leadership ability and interpersonal effectiveness.
• Ability to work autonomously and in a team environment.
• Strong communication, critical thinking, problem solving, and computer skills.

Responsibilities

• Coordinate all aspects of care for the transplant patient population.
• Assess and plan in collaboration with the patient, caregiver, and multidisciplinary healthcare team.
• Provide patient and family education on a consistent basis.
• Provide leadership through activities such as community involvement, committee work, and participation in quality improvement and research.
• Follow ANA/UNOS/FACT guidelines as the basis for practice.
