Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to manage care coordination for the transplant patient population in Minnesota. You will collaborate with patients, families, and a multidisciplinary team to plan and implement care, while educating patients and families throughout the care continuum.

Responsibilities include ensuring adherence to ANA standards, UNOS, and FACT guidelines; leading quality improvement initiatives; and coordinating with various stakeholders.

Qualifications

• Graduate of ACEN or CCNE accredited nursing program.
• RN license current per state requirements; BLS certification required.

Responsibilities

• Coordinate all aspects of care for transplant patients with the multidisciplinary team.
• Assess data and plan care aligned to patient age-specific needs.
• Provide patient and family education; lead quality improvement and committee activities.
• Follow ANA standards, UNOS, and FACT guidelines in practice.
