The role emphasizes collaboration with nursing leadership, coordination of care, and education for patients and families, with emphasis on cardiac or ICU background where possible.

Qualifications

• ASN required; BSN preferred.
• Indiana RN license or NLC required, with BLS.
• Experience in transplant programs; strong leadership and coordination.
• Strong organizational, written and verbal communication skills.
• Ability to manage multiple tasks and projects.
• ICU or cardiac care experience preferred.

Responsibilities

• Coordinate evaluation, treatment, and follow-up care for transplant patients and donors through a multidisciplinary approach.
• Collaborate with nursing leadership, primary nurses, and the healthcare team to facilitate smooth transplant processes.
• Manage multiple tasks and projects related to transplant care, ensuring timely and effective service delivery.
• Track patient care activities, document outcomes, and support quality improvement initiatives.
• Educate and support patients and families throughout the transplant process.
