Qualifications

• Baccalaureate Degree in Nursing is required.
• Masters in Nursing and certification in specialty area are preferred
• Or Baccalaureate Degree in Physician Assistant is required; Masters preferred.
• New York State Nurse/NP/PA License with current registration.

Responsibilities

• Coordinates multidisciplinary screening and evaluation of transplant patients with the team.
• Ensures lab tests and tissue typing are completed and updated.
• Communicates with active transplant candidates on waiting list status.
• Facilitates organ offers and coordinates post-offer communication with surgeons and patients.
• Coordinates inpatient and outpatient transplant follow-up visits and education on medications.
