Job summary

Mass General Brigham Incorporated is seeking a transplant coordination support professional for a 3-4 month assignment with hospital option to extend. You will be the single point of contact for facilities and providers regarding transplant planning, utilization review, and follow-up.

You will track transplant-eligible members, coordinate with care management, and communicate with patients, providers, and insurers to ensure end-to-end coordination.

Qualifications

• 2-3 years health plan clinical services/utilization management experience required.
• 2-3 years clinical experience required.
• 2-3 years transplant management expertise/knowledge required.

Responsibilities

• Ensure transplant coordination is holistic and end-to-end.
• Interact with providers, staff, and members/advocates to plan transplant activities.
• Review information and provide recommendations based on clinical material.
• Track status of transplant-eligible members to ensure comprehensive support.
• Coordinate communication between patients, providers, and insurance regarding discharge needs.
• Obtain authorizations and track each member for accurate transplant coordination.
• Participate in interdisciplinary meetings to discuss complex cases and propose solutions.
