Qualifications

• Associate's Degree Nursing required and/or Bachelor's Degree Healthcare Administration or related field of study required
• 2-3 years health plan clinical services/utilization management required
• 2-3 years clinical experience required
• 2-3 years transplant management expertise/knowledge required

Responsibilities

• Collaborate with providers, facilities, and care management teams to plan transplant activities.
• Obtain authorizations and track each member's transplant file for accuracy.
• Coordinate communication among patients, providers, and insurers regarding discharge needs.
• Monitor status of transplant-eligible members across lines of business.
