Responsibilities

• Coordinate evaluation, work-up, and listing for transplant candidates and living donors.
• Deliver longitudinal patient management for post-transplant patients in inpatient and outpatient settings.
• Provide education to patients, families, and donors; support care coordination and transitions of care.
• Participate in quality initiatives and ensure regulatory compliance (CMS/OPTN).
• Act as a clinical resource within transplant services and contribute to staff education.
