Job summary

Community Health of Central Washington is seeking a Transitions of Care Nurse to ensure safe transitions between healthcare settings and help patients manage acute and chronic health needs.

The role emphasizes clinical assessment, patient outreach, education, and care coordination to promote continuity of care within CHCW’s value-based programs and initiatives. You will follow up with patients after ED visits and hospital discharges, collaborating with caregivers and inter-disciplinary teams.

Qualifications

• Bachelor of Nursing or Associate degree in nursing plus 2 years nursing experience.
• WA RN license required.
• BLS certification required.
• Driver's license and auto insurance required.
• Immunizations and pre-employment drug screen required.

Responsibilities

• Provide follow-up after ED visits and inpatient discharges.
• Coordinate care with patients, caregivers and teams.
• Deliver patient-centered education and support self-management.
