Job summary

Community Health of Central Washington is seeking a Transitions of Care Nurse to ensure safe transitions between healthcare settings and aid patients with acute and chronic conditions. Qualifications

• Bachelor of Nursing or Associate degree in nursing with 2 years clinical nursing experience.
• WA Registered Nurse license is required.
• BLS certification is required.
• Driver's license and auto insurance are required.
• Immunizations required prior to start date (COVID, Hep B, Tdap, MMR, Varicella, TB/PPD, Flu).
• Pre-employment drug screen is required.

Responsibilities

• Provide follow-up care after ED visits and inpatient discharges.
• Coordinate transitions of care with patients, caregivers, and interdisciplinary teams.
• Deliver patient-centered education and psychosocial support.
• Support CHCW value-based programs and initiatives.
