Tanner Health is seeking a Care Coordinator RN to ensure appropriate levels of care and smooth transitions for patients. You will coordinate a multidisciplinary team to organize efficient, quality care and develop transition plans considering benefits, resources, and medical conditions.

You will apply assessment, planning, implementation, coordination, monitoring, and evaluation to drive improvements in care quality and cost-efficiency, while enhancing patient outcomes and satisfaction.

Qualifications

• Associate Degree or 2 years of college coursework.
• Three years of related experience; working knowledge of specialized practices.
• Current Georgia and Alabama RN licenses; BSN/MSN preferred.
• Knowledge of utilization review guidelines, case management principles, and regulatory standards.

Responsabilités
