Transitions Care Coordinator

The Transitions Care Coordinator facilitates seamless patient transitions from facilities to the home care environment, serving NHC HomeCare Franklin.

Qualifications:

Required:

• Current LPN or RN license in the state of service
• One year of home health or hospital case management experience
• Reliable transportation, valid driver’s license, and auto insurance

Preferred:

• Experience in home health or healthcare business development

Position Highlights:

• Assist patients in navigating post-acute care needs after selecting NHC HomeCare
• Assess, plan, coordinate, monitor, and evaluate care options to ensure safe transitions from facilities to home health
• Ensure patients have a physician and obtain necessary orders and face-to-face documentation, communicating these to the appropriate agency
