Must live in Bernalillo, Santa Fe, Valencia, Sandoval or Torrance county, NM

Role Description

Facilitates a team approach, including the Interdisciplinary Care Plan team, to ensure appropriate interventions, cost-effective delivery of quality care and services across the continuum. Collaborates with the interdisciplinary care plan team which may include the member, caregivers, legal representative, physician, care providers, and ancillary support services to address care issues, specific member needs, and disease processes whether medical, behavioral, social, community-based, or long-term care services.

Coordinates care of individual clients with application to identified populations using assessment, care planning, implementation, coordination, monitoring, and evaluation for cost-effective and quality outcomes.

Ideal Candidate
  • RN license
  • Must have a valid driver’s license
  • Experience in utilization management, home care, community health, long-term care, or occupational health
Qualifications
  • Master's degree with 4 years of experience, Bachelor's degree with 8 years, Associate's degree with 9 years, or 12 years of experience may substitute for formal education requirements.
  • Proficiency in Microsoft Word, Excel, and Outlook required.
  • Experience analyzing trends based on decision support systems.
  • Business management skills including cost/benefit analysis, negotiation, and cost containment.
  • Knowledge of referral coordination to community and private/public resources.
Responsibilities
  • Facilitates a team approach, including the Interdisciplinary Care Plan team, to ensure appropriate interventions, cost-effective delivery of quality care and services across the continuum. Collaborates with the team including members, caregivers, legal representatives, physicians, and care providers to address care issues, needs, and disease processes.
  • Conducts in-depth health risk assessments and comprehensive needs assessments covering psycho-social, physical, medical, behavioral, environmental, and financial parameters.
  • Provides care coordination for members with chronic or complex conditions requiring intensive interventions and oversight, utilizing multiple clinical, social, and community resources. Implements, coordinates, and monitors strategies to improve health and quality of life outcomes.
  • Develops and communicates plans for service authorization and acts as the point of contact to ensure appropriate service delivery, especially during transitions to home care or community-based services.
  • Conducts face-to-face home visits as required.
  • Regularly reviews and assesses care plans to identify gaps, trends, and areas for improvement, adjusting plans as necessary in collaboration with members and the care team.
  • Promotes the appropriate use of clinical and financial resources to enhance care quality and member satisfaction. Generates reports aligned with care coordination goals.
