Qualifications

Responsibilities

• Incorporate the values and principles of the Agency's mission.
• Train, supervise and work with individuals with disabilities in community, vocational, and educational facilities.
• Implement discovery-based techniques to assess where students are at their best and translate needs to move them forward.
• Provide support to students on the job (internships) by understanding and maintaining tasks and requirements.
• Compose clear and concise objective/subjective progress notes.
• Attend IEP meetings, risk management meetings and close out meetings.
• Adhere to regulations regarding DDPC, OSHA, and DOL.
• Efficient and proper use and maintenance of assigned equipment and supplies.
• Implementing components of each program service plan
• Ensure the safety of all students under supervision.

The Transitional Specialist is further responsible for ensuring that support services are consumer directed, enhance individual competencies, promote meaningful relationships and maximize individual learning potentials. Exposing students age 14-22 to skills needed to move them forward in developing a plan for their next steps in life.
