Job summary

Brooklyn Community Services is seeking a Case Manager for a 39-bed Young Adult Transitional Shelter in Sunset Park, Brooklyn. This full-time role (35 hours weekly) focuses on delivering quality services to homeless young women, guiding them toward permanent housing and independent living skills.

Responsibilities include coordinating referrals, completing plans, and providing crisis intervention. Experience with HMIS and strong communication are required.

Qualifications

• High school diploma required with 5 years homelessness experience; preferred BSW/BA in human services.
• Direct face-to-face contact with residents; strong communication is mandatory.
• Experience with CARES Collaborative Homeless Management Information System is needed.

Responsibilities

• Provide high quality program services to all clients per funder and regulatory requirements; follow BCS policies.
• Navigate client services including referrals for medical, mental health, and substance abuse services; coordinate appointments.
• Complete assessments and service plans promptly.
• Complete progress notes within 24 hours of sessions.
• Meet with each caseload at least weekly or as directed by the Program Director.
• Provide crisis intervention and timely incident reports.
• Cover caseloads during staff vacancies to maintain care quality.
• Facilitate one group per week (e.g., mental health awareness, substance abuse prevention).
• Coordinate resident move-ins and move-outs.
• Participate in community recreational events to boost social skills.
• Attend all meetings, trainings, and case conferences as assigned.
