Job summary

The Salvation Army Indiana Division is seeking a Transitional Housing Ambassador Program Counselor to develop and implement individualized treatment plans, manage client caseloads, and provide comprehensive counseling services. This full-time role requires collaboration with a multidisciplinary team, adherence to treatment protocols, and timely documentation of progress in the EMR system.

Qualifications

• Bachelor’s degree or higher in human services or related field required.
• Background checks are required and will be reviewed.
• 1–2 years direct services experience in substance abuse preferred; evening/weekend willingness.

Responsibilities

• Develop and implement individualized treatment plans from Biopsychosocial assessment.
• Provide case management as needed for assigned clients.
• Conduct individual, family, and group counseling per treatment plan.
• Teach educational classes as needed.
• Maintain confidentiality per laws and TSA standards.
• Document progress in EMR and prepare notes within 24 hours of sessions.
• Collaborate with multidisciplinary team and staff meetings.
• Ensure continuity of care and client safety across services.
• Overtime as needed to cover facility operations.
