The Lutheran Home Skilled Nursing Community in Arlington Heights, IL seeks an experienced RN to oversee the Transitional Care Unit as Nurse Manager. Lead care coordination, discharge planning, and ensure high-quality resident outcomes in collaboration with interdisciplinary teams.

This full-time leadership role requires strong communication, organizational skills, and the ability to guide staff through rehabilitation transitions and regulatory compliance, with on-call responsibilities.

Qualifications

• Current Illinois RN license
• Minimum of two years of nursing experience in skilled nursing, rehab, post-acute, or long-term care
• Experience coordinating resident care across multiple disciplines
• Strong communication and organizational skills

Responsibilities

• Coordinate care for residents receiving short-term rehabilitation and skilled nursing services
• Assist with admission assessments, care planning, and discharge preparation
• Collaborate with physicians, therapists, nursing staff, social services, and families to support individualized care
• Identify and address changes in resident condition to promote positive outcomes and reduce avoidable hospital transfers
• Provide guidance and education to residents, families, and team members regarding treatment plans and recovery goals
• Monitor documentation, care delivery, and regulatory compliance standards
• Support operational and clinical initiatives within the rehabilitation program
• Participate in an on-call rotation to assist with resident care needs
