Job summary

iCare Health Network is seeking a dynamic Transitional Care Nurse (RN) for our Meriden facility, Silver Springs Care Center. You will promote seamless transitions for patients from hospital to home, coordinating admissions, communicating with physicians, and supporting families.

The role requires an active RN license, IV certification, and ability to travel regularly to multiple iCare locations in the Meriden area. Strong interpersonal and teaching skills are essential to succeed.

Qualifications

• Current RN license (state-specific) in good standing
• IV certification required
• Familiarity with electronic health record systems (PCC)
• Exceptional interpersonal, organizational, and clinical teaching skills
• Ability and willingness to travel regularly to all affiliated buildings

Responsibilities

• Represent and promote iCare Management and Touchpoints Rehab centers to hospitals and physicians
• Identify and evaluate new patients and coordinate admissions
• Maintain strong relationships with hospitals and physicians during post-acute transitions
• Travel regularly within the community to multiple iCare locations with Meriden focus
• Coordinate and communicate patient needs prior to admission and during discharge
• Conduct patient assessments, track interventions, and educate patients and families during transitions

Transitional Care Nurse

iCare Health Network is seeking a dynamic and experienced Transitional Care Nurse (RN)to join our team. Focus will be at our Meriden facility, Silver Springs Care Center

Job Responsibilities:

As a Transitional Care Nurse your role will serve to represent and promote iCare Management and Touchpoints Rehab centers through; hospital onboarding patients, identification of new patients, and accurate clinical evaluation of patient needs. The Transitional Care Nurse is an integral part of the patient’s transition throughout the health care continuum. Successful applicants will have and/or create trusting and synergistic relationships with hospitals and physicians who are discharging post acute, skilled nursing patients. Strong knowledge of local healthcare market. Ability to travel regularly within your community to multiple iCare locations, with a focus in the Meriden area.

Additional responsibilities:

• Secure patient referrals from key healthcare organizations.
• Ability to obtain/assess quickly with new referrals
• Assess and communicate patient healthcare needs prior to admission
• Coordinate admission with the team
• Extensive communication with physicians and mid-level providers from multiple referral sources such as tertiary care centers, physicians’ offices, cardiology services and many more.
• Extensive rounding within multiple facilities
• Patient assessment and interview
• Ordering and tracking of interventions, balancing and tracking daily weights, medication changes, clinical labs monitoring.
• Assist patient and family as a resource in education as they transition through the health care continuum.
• Assist with planning and managing positive discharges and outcomes including transition to home care services.

Candidats should have:

• Current RN license (state-specific, active, and in good standing).
• IV certification required.
• Familiarity with electronic health record systems (e.g., PCC).

• Ability and willingness to travel regularly to all affiliated buildings.
