Job summary

iCare Health Network in Meriden, CT is seeking a dynamic Transitional Care Nurse (RN) to support patient transitions across hospital systems and post-acute care facilities. You will identify new patients, evaluate needs, coordinate admissions, and maintain strong relationships with hospitals and physicians.

The role requires an active RN license, IV certification, and comfort with PCC electronic health records, plus willingness to travel between iCare locations in the Meriden area.

Qualifications

• Active RN license in good standing.
• IV certification required.
• Familiarity with electronic health record systems (PCC).
• Exceptional interpersonal, organizational and clinical teaching skills.
• Ability and willingness to travel regularly to affiliated buildings.

Responsibilities

• Secure patient referrals from key healthcare organizations.
• Assess and communicate patient healthcare needs prior to admission.
• Coordinate admission with the team.
• Maintain extensive communication with physicians and mid-level providers from multiple sources.
• Extensive rounding within multiple facilities.
• Conduct patient assessment and interview.
• Order and track interventions, daily weights, medication changes and labs.
• Assist patient and family as a resource during transitions to home care.
