• Collaborates with outpatient, case management, and post-acute care providers to coordinate transitional needs
• Prioritizes referrals and follow-up activities
• Documents all patient care activities in electronic medical records
• Develops individualized plans for patients focusing on self-management and follow-up care
• Visits patients at home or in ambulatory settings as part of the Transitional Care Program
• Provides education on chronic illnesses and device use
• Maintains professional communication with patients, families, and care teams
• Participates in interdisciplinary meetings and care coordination efforts
