Qualifikationen

• Capable of maintaining accurate records and reports using computer systems.

• Receive referrals and assess eligibility for the Care Transitions program.
• Provide guidance and support to patients and families for smooth care transitions.
• Conduct in-person and telephone visits to promote self-care management.
• Identify medication discrepancies and assist reconciliation with providers.
• Educate patients to recognize warning signs and attend required appointments.
• Enter client data into PeerPlace and other data systems.
• Advise on community resources and refer for further health counseling as needed.
• Develop referrals to expand program reach and promote related services.
• Participate in supervision, meetings, and professional development.
