Qualifikationen

• Minimum 2 years in health care with discharge planning or care management.
• RN license active in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Knowledge of Care Transitions Intervention (CTI) or similar model.

Aufgaben

• Support care transitions from hospital to subsequent settings within a 30 day program.
• Collaborate with discharge planners, hospitalists, providers, facility staff, and families.
• Coordinate transitions to settings with adequate caregiving and medical oversight.
• Facilitate ICT meetings and educate non-behavioral health care managers.
• Travel up to 40-50% as required by state/contract.
