Job summary

Alliance Health is seeking a Care Manager II-Facility Based to coordinate transitional care for members with physical and behavioral health needs in acute and facility settings. You will participate in discharge planning, ensure medication reconciliation, and develop a 90-day post-discharge plan with the care team.

The role requires an active NC RN license or an advanced clinical license (LCSW/LMFT/LCAS/LCMH C/LPA) and a master's degree in a related field.

Qualifications

• Graduation from an accredited nursing program with RN license in North Carolina or Master's in Human Services with a valid NC clinical license.
• Experience in care management, case management, discharge planning, or utilization management in behavioral health or medical managed care.
• Knowledge of mental health, substance abuse, and developmental disabilities; familiarity with Medicaid requirements.

Responsibilities

• Provide Care Team support and transition planning for members moving from inpatient settings.
• Develop a 90-day post-discharge plan with the care team and educate members and caregivers.
• Coordinate with providers, housing, social services, medications, transportation, and follow-up visits within 7 days post-discharge.
