Crmchealth is seeking a Transitional Care Coach (RN) to direct, manage and monitor activities related to quality and process improvement, case management, clinical pathway development and discharge planning within the Pathways System for Case Management.
Associates in Nursing is required and a Bachelor's in Nursing is preferred. Position requires a current Tennessee RN or LPN license to practice.

- Direct and monitor quality and process improvement activities.
- Coordinate case management and discharge planning for Pathways system.
- Develop clinical pathways and oversee implementation.
