Nurses At Home in Hartford County, CT is seeking a personable Transitional Care Coordinator / Home Health Liaison to grow our presence and help patients transition from hospital or rehab to home with smooth, coordinated care.

You will build and maintain relationships with hospitals, physician offices, and referral partners, serve as the primary contact for discharge planners, and educate partners about our home health services to expedite timely admissions across Hartford County.

Competenze

• Healthcare experience preferred; home health or post-acute care background is valuable.
• Ability to develop referral relationships with hospitals and facilities.
• Strong organizational and time-management skills.
• Valid driver’s license and ability to travel across Hartford County.
