Job summary

Care Navigators On Demand is seeking a Case Manager to coordinate patient care within a hospital setting, ensuring efficient use of resources and adherence to care plans. You will support patients through assessment, planning and discharge processes, influencing outcomes and satisfaction.

The role requires licensure in nursing or social work and a strong background in acute care or ICU/CCU settings, with CCMC/ARN certifications preferred. Equal opportunity employer.

Qualifications

• Appropriate licensure in a related healthcare field is required, or commensurate experience.
• Five years of experience in acute care Medical/Surgical and/or ICU/CCU settings preferred.
• Certification in Case Management (CCMC/ARN) preferred.

Responsibilities

• Coordinate management of care for a specified patient population in a single hospital.
• Follow patients throughout the continuum of care to ensure optimal resources and compliance.
• Provide ongoing support through assessment, care planning, plan implementation and evaluation.
• Improve quality of patient management and satisfaction through integration of case management and discharge planning.
