Job summary

Advocate Health is seeking a skilled Nurse Case Manager for the Administration: Care Management department. This full-time role requires 40 hours per week, Monday–Friday 8:00am–4:30pm with rotating weekends and holidays.

Qualifications

• Licensed Registered Nurse in the state of practice

• Collaborates with patient, family, and care team to meet patient care goals.
• Initiates and coordinates case management across different care levels.
• Communicates with payers and external reviewers as required.
