Job summary

Universal Healthcare IPA, Inc. is seeking a Case Management Assistant - Transitional Care Services in Fresno, onsite.

The role supports the Case Management Department and Nurse Case Managers through coordinating services within the ECM program, focusing on transitional care across ER visits, acute care, and post-acute settings. The position requires a High School diploma or GED and at least 3 years in health care, with strong organizational and communication skills.

Qualifications

• High school diploma or GED required.
• Minimum 3 years of health care or community health experience.
• Experience with Care Management or ECM programs preferred.
• Strong communication and organizational abilities.
• Ability to handle confidential information professionally.

Responsibilities

• Assist Case Managers in coordinating post-discharge services.
• Gather clinical information and schedule follow-up appointments.
• Support care transitions and referrals with internal partners.
• Document activities in CM System and report variances.
• Coordinate transportation and provider appointments for members.
• Maintain data accuracy and confidentiality.
• Attend ICT meetings and assist with training new staff.
