Job summary

BAYADA Home Health Care is seeking Licensed Practical Nurses (LPNs) to join the Transition Team in Chester, Delaware, and Montgomery counties. This full-time role focuses on transitioning patients from hospital to home care, delivering high-quality care and training BAYADA staff.

Shifts include day and overnight options, with weekly pay and comprehensive benefits. Candidates must hold an RN license, have NICU/PICU/ICU or home care trach/vent experience, CPR certification, and be able to travel

Qualifications

• Have a current license as a Registered Nurse.
• NICU, PICU, ICU, or home care trach/vent experience required.
• Hold current CPR certification.
• Ability to travel throughout Chester, Delaware, and parts of Montgomery County.
• Availability to commit to an ongoing, consistent schedule.

Responsibilities

• Deliver quality care to clients in home settings.
• Provide training to BAYADA care team.
• Support transition of care from hospital to home care services.
