BAYADA Home Health Care is seeking dedicated Registered Nurses for our Transition Team to support seamless hospital-to-home care across Chester, Delaware, and Montgomery Counties. This full-time role offers day and overnight shifts, with extensive training and comprehensive benefits.

Qualifications

• RN license required
• CPR certification required
• Willingness and ability to travel across multiple counties
• 40 hours per week with weekend availability
• Experience in NICU/PICU/ICU or home care trach/vent preferred

Responsibilities

• Provide high-quality care to infants, children, and adults at home.
• Support transition of care from hospital to home care services.
• Deliver training to BAYADA care team and ensure continuity of care.
