Job summary

Maxim Healthcare is seeking a Licensed Practical Nurse (LPN) to join our Transition team in the Charlotte area. The Transition Team provides home-based, provisional nursing care to expedite patient discharge and ongoing recruitment of full-time home healthcare nurses.

Qualifications

• Currently licensed as an LPN in the state of practice.
• Competency with adult and pediatric patients.
• Competency with enteral feeds, tracheostomies, and ventilators.
• Current TB or Chest X-Ray.
• Current BLS card.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient�s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
