Job summary

Maxim Healthcare Services, Inc. is hiring a Licensed Practical Nurse (LPN) to join the Transition Team. This unit provides care in patients' homes during a provisional period after referral, aimed at getting patients home while primary staff are recruited.

The transition team offers training and shadowing before independent practice, with flexible PT/FT schedules and ongoing professional development opportunities. Competitive pay is provided with weekly paychecks and comprehensive benefits.

Qualifications

• Must be licensed as an LPN in the state of practice.
• Competent with enteral feeds, tracheostomies and ventilators.
• Able to care for adult and pediatric patients.
• Current TB screening or Chest X-Ray is required.
• Current BLS certification is required.

Responsibilities

• Utilize the nursing process to assess, plan, implement and evaluate patient care.
• Provide skilled nursing care/services per prescribed orders on each assignment.
• Assess physiologic and psychosocial changes and document diagnoses.
• Collect and interpret data from health care team and document findings.

Hourly Pay: $35 - $36 / Hour

Maxim Healthcare is hiring for a Licensed Practical Nurse (LPN) to work on our Transition team. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.

Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-30 days, with a 14 day grace period. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then transitioned to another new admission.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.

• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN in the state in which the LPN will practice.
• Competency with enteral feeds, tracheostomies, and ventilators.
• Competency with adult and pediatric patients
• Current TB or Chest X-Ray.
• Current BLS card.
