Job summary

Green Chimneys is seeking a Transition Coordinator at the Brewster Main Campus. This full-time role (40 hrs/week) provides comprehensive casework for transitioning RTC youth and families, coordinating with community providers from intake through return to the community.

Education: Bachelor’s Degree in Human Services with 2 years’ direct-service experience; Licensure: Valid Driver’s License. Salary: $46,020 per year. Benefits eligible and provided by the organization.

Qualifications

BROADRESPONSIBILITIES:

TheTransition Coordinator is responsible for providing comprehensive caseworkservices for transitioning RTC youth and their families/guardians. Theywill be responsible for providing community-based linkages and supports totreatment and other services beginning at the intake phase and through thetransition back to the community. Services will be provided to the RTCyouth and their families/guardians in their home communities and will becoordinated with all necessary community providers of services.

Education/Experience:

• Bachelor’s Degree in HumanServices and 2 years’ experience providing direct services to or linkingchildren with serious emotional disturbance to services.

Licensure/Certification:

• Valid Driver’s License
