Job summary

Green Chimneys is seeking a Transition Coordinator at the Brewster Main Campus. This full-time role (40 hrs/week) provides comprehensive casework for transitioning RTC youth and families, coordinating with community providers from intake through return to the community.

Education: Bachelor’s Degree in Human Services with 2 years’ direct-service experience; Licensure: Valid Driver’s License. Salary: $46,020 per year. Benefits eligible and provided by the organization.

Qualifications

• Bachelor’s Degree in Human Services and 2 years’ experience providing direct services or linking children with services.
• Valid Driver’s License is required.

Responsibilities

• Provide comprehensive casework services for transitioning RTC youth and their families/guardians.
• Coordinate community-based linkages and supports from intake through transition back to the community.
• Coordinate with community providers of services to ensure continuity of care.
