Job summary

Qualifications

• Bachelor's degree in human services or related field preferred; substitutions allowed.
• Experience with students with disabilities in transition or employment services preferred.
• Strong communication and organizational skills required.
• Knowledge of transition practices and benefits planning or willingness to learn.
• Valid driver’s license; ability to lift 35 pounds.

Responsibilities

• Guide students in exploring career options and developing workplace skills.
• Act as primary school liaison to identify eligible students and provide resources.
• Connect students and families with resources, including benefits planning and community support.
• Track and report student progress with accurate data.
• Participate in team meetings and support staff training.
