Qualifications

• Experience in hospital/healthcare settings or social work background preferred.

Responsibilities

• Facilitate safe and timely discharge/transfer to post-acute settings.
• Collaborate with care team to implement discharge plan and transitions.
• Document interventions in medical records and coordinate post-acute care.
