Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, or a related field.
• 3+ years of experience working with individuals with developmental disabilities.
• Valid driver's license and reliable transportation.
• Bilingual English/Spanish preferred.

Responsibilities

• Connect with students, families, and schools to support transition planning.
• Attend IEP meetings and provide guidance on available resources and services.
• Assist families with benefits, funding opportunities, and community supports.
• Build strong relationships with school districts and community partners.
• Advocate for individuals and help them achieve their goals for adulthood.
