Job summary

UCP Seguin of Greater Chicago is seeking compassionate, reliable Transitional Support Specialists (Mid Shift) to join our team. This full-time role offers 35-42 hours per week and involves direct support for adults with developmental disabilities, including personal care and skill development.

You will work under supervision to promote independence, inclusion, and growth, transporting participants to appointments and activities, and documenting progress per ISPs.

Qualifications

• A valid Illinois Driver's License.
• Proof of current automobile insurance.
• A reliable personal vehicle for transportation.
• The ability and willingness to transport participants safely to appointments, community activities, employment sites, and other destinations.
• The ability to obtain and maintain certifications in CPR/First Aid, Medication Administration, Defensive Driving, and Behavior Management Training.
• Strong communication, organizational, and interpersonal skills.
• A commitment to participant safety, dignity, confidentiality, and individual rights.
• The ability to work independently while collaborating effectively with participants, families, guardians, healthcare providers, and team members.

Responsibilities

• Assist participants with activities of daily living, including personal care, meal preparation, medication administration, housekeeping, mobility, and community engagement.
• Support individuals in developing independent living, vocational, communication, recreational, and social skills while encouraging community inclusion and self-determination.
• Implement ISPs, document participant progress, and maintain accurate records.
• Participate in team meetings and ensure compliance with agency policies and safety procedures.
