Job summary

Metrocare Services seeks a QMHP-C&A in Dallas to provide case management and recovery planning for children and adolescents with serious emotional disturbance. You will work with an interdisciplinary team to coordinate services and monitor progress to improve community functioning and reduce hospitalizations.

Responsibilities include assessment, goal setting, service linkage, crisis intervention, and documentation. A bachelor’s or master’s in human services with relevant experience is required.

Qualifications

• Bachelor's degree in human services with 1-3 years in MH/MR, or Master's in human services with 0-2 years.
• Proficient in MS Office (Word, Excel, Outlook, PowerPoint).
• Valid Texas driver's license or eligible in your state; must obtain Texas license if relocating.

Responsibilities

• Provide case management services to children and adolescents with serious emotional disturbance.
• Coordinate services with interdisciplinary treatment team.
• Ensure authorization for clinical services.
• Develop measurable objectives and goals based on assessments.
• Document progress and follow up with individuals and caregivers as needed.
• Review service plans every 90 days or as indicated.
• Engage in crisis intervention and linkage to resources.
• Support inpatient admissions when required; assist in discharge planning.
