Qualifikationen

• Bachelor's degree in human services with 1–3 years experience in MH/MR or Master's degree with 0–2 years experience in human services.
• Strong assessment and documentation skills.
• Ability to work with diverse populations and families.

Aufgaben

• Provide psychosocial assessment on new admissions and develop recovery plans.
• Coordinate referrals, linkage, and advocacy with an interdisciplinary team.
• Engage individuals in services, crisis intervention, and continuity of care.
• Document progress and ensure timely follow-up and contact attempts.
• Support home visits and community integration as needed.
