Qualifications

• 1–3 years of mental health related experience; or Master's with 0 years.

Responsibilities

• Provides overall housing case management, advocacy, and linkage with services.
• Completes psychosocial assessments and develops housing service plans.
• Delivers skills training for budgeting, self-care, and tenancy.
• Participates in interdisciplinary team staffing and discharge planning.
• Maintains documentation and follows up on crisis interventions.
